Use four columns in this order: old_url,new_url,reason,approved. Enter full HTTP(S) URLs consistently, or use paths consistently for one site. Keep query strings when they matter. Use yes or no for approval. A blank destination stays visible for a decision; this tool never guesses a match or installs a redirect.

Values beginning with spreadsheet-sensitive characters receive a leading apostrophe. Review the file when opening or re-saving it in a spreadsheet app; escaping behavior varies between applications.
